ACCOMMODATION


Coganoi Bay Hotel is an annex of the traditional Japanese style hotel "Coganoi" and built as a western-style resort hotel. This newly built annex is located on a hill, over-looking the ocean. It promises guests a relaxing time, offering various facilities such as in-door swimming pool, spas, fitness center, garden filled with seasonal flowers, restaurants, and bars. Check-in is at 14:00 and check-out is at 12:00.

A block of sleeping rooms has been reserved for the FOM2000 participants at Coganoi Bay Hotel, which is the site of conference. The hotel offers the special discounted rates from April 9, 2000 to April 13, 2000 for forms received before April, 1 2000. Convention rates will not be guaranteed after the reservation due date. All rates below are for one person per room night and are subject to 10% service charge and 5% tax.

All rooms will be assigned on a first come, first serve basis. Arrangements for accommodations must be made through Coganoi Bay Hotel and NOT with FOM2000 Secretariat. Hotel Accommodation Forms will be processed by Mail or FAX (+81-6-6227-0558). Telephone calls ARE NOT accepted.

Payment:
A total amount of Hotel Accommodation fee is required for reservation. Coganoi Bay Hotel only accept payments by VISA and Master cards which will be charged after forms received. Forms without credit card information ARE NOT accepted.

Confirmations:
Coganoi Hotel will send a confirmation. Review it carefully for accuracy. If you do not receive a confirmation via e-mail or fax within 14 days after sending request, please contact via e-mail at fomhotel@ap.eng.osaka-u.ac.jp.

Refund Policy for Accommodation:
Written notice of cancellation is required to be sent to Coganoi Bay Hotel in advance of arrival. Cancellations made prior to February 29 will receive a full refund. Cancellations after February 29 will be charged a 50% of the first night's room fee. A full amount of the first night's room fee will be assessed after March 31, 2000. Please note that a processing fee will be charged on all the refunds for 2,000 yen.
